INSURANCE

I am an in-network provider with Premera and LifeWise through my membership with Shelterwood Collective. If you’re planning to use insurance, contact your insurance and use the prompts below to ask and confirm if your plan includes coverage for the type of care I offer.

Questions to Ask Your Insurance Provider:

  • What are my mental health benefits?

  • Does my plan have a deductible? If so, how much is it, and has any of it been met this year?

  • What is my copay or coinsurance for mental health visits?

  • Who administers mental health claims in Washington State?

  • Is Shelterwood Collective an in-network group with my plan?
(Shelterwood Collective is the group practice I belong to. Their TIN is 47-4860247 and NPI-2 is 1538663604.)

  • Does my plan cover services provided by associate-level clinicians?
(This is often linked to my individual NPI number, but it’s a good idea to ask directly.)

Please Note: Shelterwood Collective will verify your insurance benefits after scheduling. I strongly recommended that you understand your own coverage including copays, deductibles, out-of-pocket maximums, and other details. For the most accurate information, call the member services number on the back of your insurance card.

NO SURPRISES ACT

As of January 1, 2022, the No Surprises Act ensures that all clients have the right to receive a Good Faith Estimate for therapy services. This written estimate outlines session fees and projected costs so you can plan your care with clarity. You’ll receive your Good Faith Estimate when we begin paid services, as part of my intake process.
